Please bring a copy of your pets vaccination records with you or have your veterinarian fax us a copy prior to your arrival to avoid any delays during check in.

ANIMALS THAT HAVE NOT BEEN VACCINATED AT LEAST 72 HOURS PRIOR TO ARRIVAL WILL NOT BE ACCEPTED UNDER ANY CIRCUMSTANCES.

We are a flea free facility!! We administer a Capstar tablet to all pets lodging with us at a charge of $5.00 per visit. This is not an optional item and more information about it is available upon request.

The Legal Stuff ( Our lawyer made us do it!)

The following represents a contract between My Second Home and the Owner of the pet whose name appears on this Application and Facility Agreement (the "Agreement"):

By signing this Agreement and leaving his/her pet with My Second Home, Owner certifies to the accuracy of all information given in the Agreement.

Owner agrees to pay the rate for boarding, grooming, daycare and any other requested services in effect on the date pet is checked into My Second Home.

Owner further agrees that the pet shall not leave My Second Home until all charges are paid to My Second Home by Owner. My Second Home shall have and is hereby granted, a lien on the pet for any and all unpaid charges resulting from boarding pet at My Second Home. The Owner hereby agrees that in the event the boarding charges are not paid when due in accordance with this Agreement, My Second Home may exercise its lien rights upon ten days written notice given by My Second Home to Owner by certified mail to address shown on this Agreement. My Second Home may sell the pet for any and all unpaid charges, at private or public sale, in the sole discretion of My Second Home, and Owner specifically waives all statutory or legal right to the contrary. If such sale shall not secure a price adequate to pay such cost of boarding or other charges, plus cost of sale, then Owner shall be liable to My Second Home for the difference. All monies realized by My Second Home at such sale, over and above the charges due and costs of sale, shall be paid by My Second Home to Owner.

At My Second Home the security and care of your pet is the cornerstone of our business. For your protection, only the Owner or the person listed as the authorized pick up person will be able to pick up the pet. Please list any other persons you authorize to pick up your pet below:

I agree that if I pick up my pet after 12:00 pm for any reason whatsoever, I will be charged for a full day of boarding for that day. No after-hours pickups will be permitted. Additionally, there are no drop offs or pick ups on New Year s Day, Easter Sunday, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas eve or Christmas day. There may be days when we close at noon prior to a major holiday.

My Second Home Offers a large variety of toys designed to work with our sanitation system at excellent value, however, should your pet have a favorite toy, you are welcome to bring it along. We cannot however be help responsible for lost or damaged articles. Please take leashes, crates and other articles home with you at check in, as we do not have the space to store customer articles. Thank you for your understanding.

Comfortable, clean, bedding is provided by My Second Home and bedding from home is highly discouraged. My Second Home cannot be held responsible for lost or damaged bedding material.

My Second home provides a variety of veterinary prescribed diets for your pet while in our care. Should you choose to bring your own food, please apportion the food in individual servings by meal.

We are pleased to administer any medications your pet may be required to take a charge of $1.50 per occurrence.

The Owner agrees to be solely responsible for any and all acts or behavior of the pet while it is in the care of My Second Home. Pets exhibiting aggressive behavior will be respectfully requested not to return.

I understand the conditions for boarding my pet in My Second Home and by signing this release and waiver I am agreeing to hold My Second Home, its owners and employees harmless from any and all claims for damages or injuries suffered by, or in any way relating to my pet's stay at My Second Home.

While we do our very best to care for your pet, My Second Home cannot guarantee the health of my pet during their stay. Some conditions are unavoidable in boarding facilities such as but not limited to weight loss, hair loss, upper respiratory infections, bronchitis, diarrhea, and parasites. I understand that all pets admitted must be protected against communicable contagious diseases and must be free of parasites. This will help to provide for a healthier environment for all.

If any problem or emergency develops and it is necessary to treat your pet, please initial one of the following directives:I agree to be responsible for any and all charges incurred to care for my pet s health per my choice.
